Healthcare POA forms for all documents is a cost of is $99 for up to 5 notarized signatures. As long as all documents are prepared and ready for signature at the same visit. For blank loose forms there is a $8 fee as well as the signing fee The above fees are cash payment discounted fees. If you are paying by debit or credit care there is a $4 added seice fee. No personal checks, business checks are accepted and Invoicing for real estate transactions only. Note: all fees are subject to a 30 mile driving distance for the notary, Milage after that first 30 miles will be added at a rate of $.625 per mile
